Guidance Fees
Certificate of
Attendance (Drivers License and Drivers
Permit)
- Complete the
request form
- Charge is attached to the certificate and
will increase by $1.00 for each request,
even if the certificate is not picked
up. (Example: first request =$1.00; 2nd
request=$2.00; etc.)
- Certificate
may be picked up 24 hours later or the following
school day
- Certificate
is only good for 30 days and will have to be
requested again if it has expired; fee will
follow the above cost

GPA
- Complete

& Password
- Complete the request form
- Letter
